This occurred if Known Folder Redirection (KFR) had been previously enabled, but files remain in the original "old" folder location vs being moved to the new, redirected location.

You can install the relevant Windows 10 cumulative update on your machines through Settings Update and Security Windows Update Check for updates. This is the feature that lets you move a known folder like C:\Users\Name\Downloads to D:\Downloads, for example.

Last week, Microsoft put the brakes on its Windows 10 October 2018 version 1809 update after some reports of post-update data loss.

Microsoft's "Windows 10 Update History" page also contains similar advice for affected users.

Microsoft released the Windows 10 October 2018 Update to "seekers" on October 2 during its Surface event, completely skipping the Release Preview ring of the Windows Insiders program.

The user configured one or more of their Known Folders (Desktop, Documents, Pictures, Screenshots, Videos, Camera Roll, etc.) to be redirected (KFR) to another folder (device drive, external drive or a OneDrive folder).

Microsoft responded on October 4th by pulling the update for further investigation into the bug. "That change, combined with another change to the update construction sequence, resulted in the deletion of the original "old" folder locations and their content, leaving only the new "active" folder intact". During this process the system prompts the user and asks if they would like to move the files to the new location.

The fixed versions of Windows are getting floated in Microsoft's Windows Insider Program, which is a program for software testers. "We expect this will allow us to better monitor the most impactful issues even when feedback volume is low", Microsoft says.

Microsoft enabled new feedback functionality today that adds options to classify impact and severity of the reported issue.
